White Pine/Gracewood Senior Living is seeking a full time Assisted Case Manager to serve as the primary point of contact for families, providing personalized support to navigate and simplify their financial needs. This position involves collaborating with various financial institutions, managing transactions, and offering tailored solutions to address families’ unique financial situations.
Pay Range: $26 hourly
Key Responsibilities:
• Resident Eligibility and Enrollment
o Assist current and prospective residents in applying for and maintaining eligibility for Minnesota’s EW and CADI programs.
o Work with case managers and county financial workers to ensure residents can access financial resources.
o Assist families in navigating the enrollment process for various funding opportunities.
• Support and Coordination
o Inform families about available financial products and services that may benefit them.
o Serve as the primary liaison between clients and financial institutions, ensuring effective communication and understanding of client needs.
o Address and resolve client concerns promptly, ensuring a positive and seamless experience.
o Educate residents and families about Medicaid benefits, service options, and waiver requirements.
o Coordinate reassessments, service plan updates, and changes in care needs.
• Financial and Billing Management
o Engage in the billing and reimbursement processes for the EW services.
o Monitor resident service authorizations and funding status to ensure ongoing eligibility and proper payment.
o Collaborate with the accounting team to address billing discrepancies and ensure prompt payments.
o Assist families and residents with spenddowns, financial eligibility assessments, and Medicaid renewals.
Qualifications:
• Associate’s degree in social work, healthcare administration, or related field is preferred.
• 1-3 years of experience in senior living, long-term care, Medicaid billing, or social services.
• Must possess knowledge of Minnesota’s waiver programs, Medicaid processes, and DHS regulations.
• Experience with electronic health records and Medicaid billing systems is preferred.
