Coleman Health Services is a nationally recognized not-for-profit provider of behavioral health and rehabilitation programs to foster recovery, build independence, and change destinies for individuals, families, and businesses in our community. We are seeking an experienced Case Manager to join a multidisciplinary team of providers who conduct assertive outreach in the community with individuals with severe and persistent mental health issues (SPMI) in Stark County, Ohio. Assertive Community Treatment Team (ACT) is an evidence-based program that engages with clients who are most at risk. The ACT team assists clients with accessing community-based resources, housing, employment, making appropriate referrals and assisting with maintaining scheduled appointments.
Must have BA/BS in mental health or related field and be passionate about working with our clients; have or willing to get a NPI and an Ohio Medicaid number; have and maintain an excellent driving record; be able to work a flexible schedule including an after-hours on-call rotation; and have favorable drug screen results. This position involves working in community settings, homes, apartments, etc.
