Adult Case Manager - Stark

Coleman Health Services, a nationally recognized not-for-profit provider of behavioral health and rehabilitation programs, is seeking Client Care Coordinator / Case Manager to join our team. All Care Coordinators / Case Managers assists at risk clients with accessing community based resources, housing, employment, making appropriate referrals and assisting with maintaining scheduled appointments. This position is eligble for a $2,000 Sign-On Bonus.

Qualified candidates for Care Coordinators / Case Manager

• Must have a BA/BS
• Preferred to have 1-year experience in case management, mental health or a related field
• Must be eligible to obtain an NPI number and an Ohio Medicaid number for billing purposes, which Coleman can assist you in obtaining.
• Must have and maintain an excellent driving record and vehicle for transporting clients in the community.
• Candidates will be expected to complete a drug and BCI screening within 48 hours of an offer of employment.
