What are the responsibilities and job description for the Youth Case Manager - Canton, OH position at Coleman Health Services?
We are seeking a passionate individual to provide short term case management services to youth and their families in their natural environment, schools, and various community settings. An ideal applicant will have passion for making a difference in the lives of the youth and their families.
Duties include:
- Provide short-term, community-based case management to assigned families following a behavioral health crisis
- Safety planning and increasing healthy coping skills
- Develop treatment plans
- Assist with accessing supports, resources, and outpatient services that will help stabilize the crisis and keep them safe in the community
- Function as part of an interdisciplinary team and develop and maintain effective relationship with community professionals
Qualifications:
- Bachelor’s degree in behavioral health or related field
- Must be eligible for an NPI number and an Ohio Medicaid number
- Acceptable driving record
- Must be willing to complete a background check and drug/alcohol screening if selected for the position
Preference given to the following candidates:
- Prior case management experience
- Understanding of child and adolescent development, behavioral health, and community resources
- Past or current experience in motivational interviewing, crisis intervention, and trauma informed care
