Coleman Health Services is seeking a Case Manager for Portage County, Ohio. We are seeking a Case Manager to provide care coordination and other case management services to individuals in Portage County who have mental health and substance use disorders.

Duties include:
- Coordinate ongoing care with internal and external healthcare providers
- Assist with symptom monitoring and coping skill development
- Assist with making healthcare, service, and community resource referrals
Qualifications:
- BA/BS in mental health/related field or three years' related experience
- Have/obtain a NPI number and an Ohio Medicaid number
- Have/maintain an excellent driving record
Preference given to the following candidates:
- BA/BS in mental health/related field
- Someone with a CDCA
- Experience in case management
- Experience working with adults in a community mental health setting
