Collective Health is looking for a full time, experienced, and forward - looking Licensed Clinical Social Worker to guide and support members facing complex conditions driven by data, shaped by evidence-based best practices, and supported by world-class customer service. The SW Care Managers are part of an interdisciplinary care team located in Collective Health’s Lehi, Utah office. The SW Care Manager will ensure that our members’ care is optimally coordinated.

The actual pay rate offered within the range will depend on factors including geographic location, qualifications, experience, and internal equity. What You'll Do
We are looking for an expert Licensed Clinical Social Worker who is adept at navigating people - to - care resources and managing complex medical conditions. The Social Worker would be responsible for providing telephonic care management including, but not limited to:
- Lead outreach efforts to high-risk individuals with complex medical conditions through telephonic or electronic outreach
- Complete telephonic social work assessments and care plans for patients with behavioral, social, financial and environment problems
- Educate and empower members using decision support through multiple modalities including telephone and email to help members achieve their health goals
- Identify and discuss current barriers related to member’s current condition and provide interventions to address these barriers
- Provide emotional support, behavioral health navigation, and assist with benefits
- Coordinate a range of services, including community resources and health benefit resources
- Utilize motivational interviewing to assist members to become self sufficient in their care planning
- Participate in clinical interdisciplinary rounds
- Work collaboratively on co-managed cases with nurses, pharmacists, and dietitians
- Work cross functionally with other teams to assist with benefits and claims questions
- Spend 85% of your time in the direct case management role and 15% on project work and other opportunities as they are available
- Have an active LCSW
- Have 3-5 years experience in case management; experience at a payer is a plus
- Bilingual (Spanish/English) Highly preferred
- Are passionate, empathetic, and excited to help people
- Are independent, tenacious, and capable of identifying and implementing best practices
- Are capable of thinking creatively to implement care navigation solutions that challenge status quo
- Are driven, able to carry a caseload and prioritize to complete daily tasks and outreach while meeting operational expectations
- Are tech savvy; able to talk, type, and navigate an electronic chart with ease
- Are proficient in Google suite, such as Google Sheets, Docs, etc.
- Are okay with working in a telephonic social work role (no direct/face to face patient contact)
- Are okay with a role that does not involve providing individual therapy
- Are a problem solver using best practices to achieve goals
- Are comfortable with ambiguity and open to collaborating with people across different teams
- Have experience at a health technology startup
- Experience working with diverse patient populations, including LGBTQ
- A strong moral compass and commitment to Collective Health’s core values of integrity, transparency and trust
- Additional Case Management Certification is a plus
- Discharge planning, complex care coordination, behavioral health, and/or home health experience preferred
- Oncology, behavioral health, pediatrics, and/or maternity experience a plus
