Work is performed under the general supervision of the Program Administrator with broad latitude for the use of initiative and independent judgment. Works as a part of a team managing a caseload with the goals of obtaining positive results regarding outcomes set by the person centered process. Responsibilities include: Assessing, evaluating and identifying the needs of the individual. Developing, coordinating and monitoring individual’s plans to insure quality of life and community participation that is acceptable to the individual. Locating and coordinating services and supports to prevent and/or manage crisis. Completing documentation of all assessment, planning, monitoring and crisis activities. All activities are completed in a manner that supports the mission of LifePath Systems and the principles of the specific program (Non-Waiver, HCS, TXHML) by putting individuals and families at the center of planning and service delivery. Must be able to work closely with all team members and take initiative in completing complex tasks, projects and other duties as assigned.
Additionally:
• Assess:
o Evaluate and identify the wants and needs of the individual.
o Using the Person Directed Plan, CFC plan when necessary and other information gained from ongoing interaction and interviews with the individual and those that know them best as the primary assessment tools. SC will identify priority outcomes and needs.
o All assessment activity will occur in a person/family centered manner.
• Planning and Coordination:
o To identify, arrange, advocate and collaborate with family, friends, community resources and providers to support satisfaction and progress toward person directed outcomes.
• Monitoring:
o To assess the effectiveness of the strategies and supports identified in the plan to support and individual’s progress toward an outcome and satisfaction toward a service. To advocate and adjust supports when progress is not satisfactory or satisfaction not obtained.
• Crisis Prevention and Intervention:
o To coordinate services and supports to prevent and/or manage crisis situations as identified by individuals receiving services.
• Production Data:
o To perform the tasks of a Service Coordinator while also meeting performance standards for the position.
Qualifications
Must have at least one of the following-
- A bachelor's or advanced degree from an accredited college or university
- An associate degree in a social, behavioral, human service, or health-related field including, psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, education, and criminal justice
- A high school diploma or a certificate recognized by a state as the equivalent of a high school diploma, and two years of paid or unpaid experience with individuals with intellectual or developmental disabilities
