Description
- Performs psychosocial rehabilitation services based in trauma-informed and person driven care model to individuals in the most intensive level of care. Provides person directed case management to assist in navigating access to medical, social, vocational, financial, educational, and other individualized services. Provides person directed psychosocial rehab to develop coping and other skills needed to feel successful in their home and community. The case manager role utilizes community partners and resources to expand an individual’s access and engagement to appropriate holistic behavioral healthcare. Promoting choice and autonomy through a strength-based model is necessary to assist individuals served with accessing social determinants of health. Integrity is necessary to ensure adherence to the Health and Human Services Commission, Texas Administrative Code, contracts, grants, and Lifepath Systems policies and procedures. Documentation which meets fidelity standards is expected. Working independently and collaboratively to foster better outcomes for individuals served and community partners, to facilitate improvement in measurable ways, is central to LifePath Systems mission to serve. Facilitating community integration by understanding and implementing the Assertive Community Treatment model is a large portion of day-to-day responsibilities. Reliable transportation and valid Texas driver license required. Other duties may be assigned to meet the needs of the program and individuals served.
Qualifications
- Bachelor’s degree in psychology, social work, or related field from a four-year college or university.
- At least two years of experience in working with the community, preferred. (i.e., social services, recovery, customer service, sales).
- Bilingual (Spanish/English) preferred.
- Knowledge and experience in providing case management, skills training, and symptom management services to individuals with mental illness.
- Ability to assess individual needs, to coordinate services, and to communicate effectively.
- Knowledge of the social services system and the ability to connect individuals with appropriate services.
- Ability to write clearly and professionally.
- Ability to think clearly and logically in order to deal with potentially dangerous and/or emotional situations.
Salary : $42,500 - $53,000
