Description
This is a field-based position. The QMHP-CS will be responsible for facilitating the continuity of care for individuals served by LifePath System's behavioral health department, including coordination between state and local psychiatric facilities. Tasks include: Providing requested records to appropriate facilities, scheduling aftercare appointments for individuals upon discharge from local hospitals, completion of full intakes for individuals coming into services to include biopsychosocial, financial assessment, needs assessment, recovery plan and facilitation of initial diagnosis. Also to complete update of assessments or documentation needs for established individuals. Facilitation of referrals and linkage to additional services as needed, collecting, and recording data needed to evaluate performance measures. This position will work closely with hospital staff and other stakeholders in the community and will be expected to maintain professional relationships with all collateral contacts. Applicants must possess the ability to complete concise but detailed progress notes on all services provided. The goal of this position is to link Collin County residents with appropriate Behavioral Health Services. Must be able to take initiative in completing complex tasks and projects. Works under moderate supervision with limited latitude for the use of initiative and independent judgment.
Qualifications
- Bachelor’s degree in psychology, social work, nursing or related field from a four-year college or university that meets the Texas Administrative Code for requirements for a Qualified Mental Health Professional- Clinical Services (QMHP-CS).
- May have a Master’s degree in counseling, social work, nursing or related fields.
- Must have at least three years of clinical experience in the treatment of individuals with mental illness and/or substance abuse disorders.
- Experience in service coordination including community resources.
- Ability to foster collaborative relationships with key interfacing teams, be a self-starter who works closely with all team members and takes initiative in completing tasks.
- Ability to ensure effective communication with the multi-disciplinary team and across a variety of community resources.
- Will work under general supervision with moderate latitude for the use of initiative and independent judgement as well as identifying when a licensed professional need to be consulted.
- Knowledge of the social services system with the ability to connect individuals with appropriate services.
- Must be organized, detail oriented, computer proficient, and able to write clearly and professionally.
- Ability to respond with individuals and staff with patience, empathy, and understanding.
Salary : $45,500
