Description
Triages individuals seeking outpatient services to timely and effectively navigate appropriate service options available. Provides initial screenings, crisis interventions, and determines clinical and financial eligibility via telephone, zoom, or face-to-face. Completes detailed documentation in a timely manner on all services provided. Develops person-driven recovery plans and recommends appropriate level of care. Links individuals to appropriate natural and community resources to meet ongoing needs, as necessary. Ensures all services provided are compliant with TAC requirements/rules. Knowledge and adherence to trauma-informed perspective is required. This person must be efficient in EHR, have ability to multitask, and must be able to provide exceptional customer service in a busy setting. This position will provide support to the Access Center as needed.
This key role requires a team player to work in a constantly changing busy environment. This position requires accuracy, attention to detail, initiative and a proactive approach to duties and responsibilities and will work with individuals in services, the clinical team, and co-workers. The qualified applicant must demonstrate a positive attitude, considerable latitude for independent judgment, as well as professional ethics, appearance, and conduct.
Qualifications
Education, Training and Experience (including licensure and certification)
• Bachelor’s Degree in psychology, sociology, or related field from an accredited college or university as required to qualify as
a Qualified Mental Health Professional (QMHP).
• Experience working in mental health preferred.
• Experience in crisis response preferred.
• Bilingual English/Spanish preferred
Knowledge, Skills, and Abilities
• Demonstrated ability to foster collaborative relationships.
• Excellent knowledge of telehealth platforms with the ability to engage via multiple platforms as well as guide others in
telehealth set-up.
• Self-starter who works closely with all team members and takes initiative in completing tasks.
• Demonstrated ability to ensure effective communication regarding behavioral health needs and services.
• Knowledge of the social services system and the ability to connect individuals with appropriate services.
• Organized, detail-oriented, and computer-proficient with the ability to write clearly and professionally.
• Ability to respond to individuals with patience, empathy, and understanding.
• Excellent communication and listening skills.
• Performs other duties as assigned.
Salary : $42,500
