Main Responsibilities:
- Providing expert care and support to patients with communication, cognitive, and swallowing disorders.
- Working closely with other healthcare professionals to ensure seamless care coordination and communication.
- Developing and implementing individualized plans of care that address the unique needs of each patient.
- Providing education and training to patients, families, and caregivers on communication and cognitive strategies.
- Participating in quality improvement initiatives to enhance patient outcomes and satisfaction.
