Here is what the position will look like. The CM Manager is responsible for programmatic implementation of care management services/activities according to contract deliverables across Colorado Access programs. This includes but is not limited to workflow design, clinical documentation, monitoring outcomes and reporting, and sharing best practices amongst teams. Ensures care management teams meet the organizational and departmental strategic goals and supports the implementation and reporting on these activities. Assumes responsibility for specific projects, pilots, grants, and other special programs related to care management, including planning, implementation, communication/coordination, and reporting components. Responsible for the recruitment/hiring, management, communication and operational workflow of the teams managed. Manage escalated member and stakeholder concerns professionally to ensure seamless operations., including interacting with clinical leadership to support staff and members. Develops and evaluates operational excellence goals related to quality and process improvement procedures such as phone monitoring, documentation review, productivity metrics, etc. Participates in continuous quality improvement of CM programming and recommends and manages improvements to automate processes, improve stakeholder satisfaction and meet COA objectives key results (OKR’s), COA organizational goals, and contract deliverables and performance measures. Participates and supports preparation for  formal auditing conducted by the Colorado Department of Health Care Policy and Financing (HCPF), Health Services Advisory Group(HSAG) and other auditing bodies. Supports internal and external partnership programs (Contracts, MOUs, cross-departmental initiatives etc. ) and provides ongoing status reports to leadership and communicates successes and issues appropriately. Must have the ability to work with both internal and external departments and service providers which could include; hospitals, medical providers, community advisory committees, outreach programs, county agencies, community resource programs and agencies that work in long term care or healthcare related programs. Works collaboratively with other department leaders and internal stakeholders to support organization strategies, goals and deliverables. Support COA and CM department at external in-person meetings and community events and meetings.       What you will bring:   Education: Bachelor’s degree in human services field required; or an equivalent combination of education and experience. Master’s degree in healthcare field preferred.     Experience: Five years of care coordination experience and minimum of two years of supervisory experience is required. Experience in managed care or healthcare programs (Medicaid/Medicare) in the public or private sector preferred.    Knowledge, Skills, and Abilities: Ability to manage and mentor teams of direct reports and multidisciplinary workgroups; requires individual who can work well independently or in workgroups.  This position requires complex critical thinking skills for strategic implementation of agency and programmatic Key Performance Indicators (KPIs), care management staff and program oversight, and to facilitate quality outcomes and resolutions.  Ability to manage internal and external relationships with stakeholders and may require internal/external presentations on various care management related topics.  Demonstrates support for the company’s mission, vision and values.  Position requires excellent written and verbal communication skills, strong time management skills, strong interpersonal skills and a strong working knowledge of technology systems that supports care management work. Ability to work with other COA leaders and departments to achieve key organizational goals and objectives. May be required to manage multiple priorities and projects with tight deadlines.  Project management and program development experience strongly preferred; Health care and quality improvement and/or regulatory experience desired.      Licenses/Certifications: Active Clinical Licensure (RN, LCSW, LCP, etc.) is preferred but not required.  A valid driver's license and proof of current auto insurance will be required for any position requiring driving.   Together we will be: an innovative and collaborative team who supports each other, the employees and vision of the company to reach our goals individually, together and as an organization.     Pay, Perks and Benefits at Colorado Access: The compensation for this position is $88,700.00 -  $108,900.00 annually. The pay rate/salary is commensurate with experience.    In addition to being part of a mission driven organization serving our community, as an eligible Colorado Access employee, you’ll receive a generous benefits package, that includes: Medical, dental, vision insurance that starts the first day of the month following start date.  Supplemental insurance such as critical illness and accidental injury.  Health care and dependent care flexible spending account options.
