Position Summary:
The Prevention Services Coordinator plays a critical role in advancing harm reduction initiatives within the community. This position involves direct service to individuals affected by substance use and related health risks, providing education, outreach, and harm reduction resources. The Coordinator will work closely with community partners and staff to promote healthier lives and connect individuals to essential services.
Key Responsibilities:
- Syringe Access & Harm Reduction Services:
Support the operation of our syringe access program both on-site and in the community, including safe disposal, distribution of harm reduction supplies, overdose prevention education, and data entry. - Community Outreach & Engagement:
Assist with mobile outreach, pop-up services, and tabling events throughout Weld County. Build relationships with community members, including people who use drugs (PWUD), sex workers, local businesses, and other key groups. - Educational Presentations & Interventions:
Facilitate educational sessions in the community, including group and individual harm reduction interventions. Represent CHN at community events and meetings, sharing knowledge on overdose prevention, HIV, HCV, and STI issues. - Referral & Support Services:
Develop and maintain a network of resources for PWUD and other high-risk populations. Provide linkage to care (LTC) and assist individuals in accessing services such as HIV/HCV/STI testing, substance use treatment, and other relevant support. - Training & Volunteer Support:
Assist with training interns and volunteers in harm reduction strategies, including syringe exchange, HIV/HCV/STI testing, and risk reduction counseling. - Testing & Counseling:
Administer HIV, HCV, and STI testing and provide individualized counseling to at-risk populations, including PWUD, people living with HIV/AIDS, members of the LGBTQ community, and those experiencing homelessness. - Collaboration & Teamwork:
Work closely with the Prevention Services team and statewide partners to develop and support culturally competent, client-centered programs. Participate in retreats, team meetings, and ongoing training opportunities. - Flexible Hours:
The position requires flexibility, with occasional evening and weekend hours for outreach events, trainings, and community engagement.
Qualifications
Preferred Qualifications
- Demonstrated creativity, initiative, and commitment to pursuing new ways to improve and expand services strongly preferred.
- Desire to help develop and grow the sexual health pilot program and/or Harm reduction program in Greeley
- Oral and written fluency in English and oral fluency in Spanish preferred.
Education:
Bachelor's Degree in Social Work, Human Services, Psychology, Sociology, Public Health, a related field, or equivalent experience required.
Software Utilized:
Experience working with basic office and database computer programs, including familiarity with Microsoft Outlook, Word, Excel, Access, SharePoint and social media outlets required.
Knowledge, Skill and Ability:
Previous experience in the delivery of evidenced-based programs is highly preferred, as well as excellent communication, client services, and data collection skills. The appropriate person will be highly organized and detail-oriented. This position requires personal and professional accountability; self-management and initiative; dedication to quality improvement; professional integrity; research ethics, and cultural competency. Must demonstrate accuracy, efficiency and superior organizational skills. Must demonstrate effectiveness in the following areas: task and time management; teamwork; and communication with teams, managers, clients and participants, and company consultants.
Material and Equipment Directly Used:
Works with standard office equipment including computer work station, printers, copiers, scanners and fax machine. Works with word processing software, spreadsheet applications, and presentation software. Communicates with clients and vendors using the telephone, e-mail, the Internet, and fax. Valid Colorado Driver’s License/auto insurance and reliable transportation required.
Physical Demands and Work Conditions:
Works in an office environment. Regular weekly and monthly reimbursable travel required. Requires clear speaking ability and the ability to use a telephone for extended periods. Requires ability to view a computer screen (near acuity). Requires finger agility for keyboarding and using a telephone and other office equipment.
