Columbia Community Mental Health is a nonprofit agency and Certified Community Behavioral Health Clinic that provides services to individuals with mental, addiction and developmental disabilities. We are looking for a qualified Care Coordinator to join our team!

Position: Care Coordinator

Department: Outpatient Behavioral Health Services (OBHS)

Shift: Mon-Fri 8:00 am to 5:00 pm (60 min unpaid lunch)

Salary Range: $56,804.80 annually - $73,444.80 annually. We use a bona fide factor system to determine starting wage (years of experience, education, credentials, and specialty skills that relate to the job requirements).

Position Requirements: requires a QMHA: a bachelor's degree in a behavioral science field from an accredited college or university or three years of full-time professional experience working in the mental health related field. Note: All QMHA positions in the state of Oregon will need to be filled by candidates who are officially QMHA certified or registered with MHACBO. For details, go directly to MHACBO (www.mhacbo.org). Position requires driving in the community.

Summary: This position works with a team of case managers within the Adult OBHS department. This team provides behavioral interventions and treatment within the scope of the Qualified Mental Health Associate credential to clients enrolled in mental health services at CCMH. Services include but are not limited to case management, skills training, psychoeducation, care coordination, and advocacy. Group and individual treatment modalities are utilized. This position doesn't provide direct supervision and functions as a member of the OBHS leadership team.
