The Clinical Social Worker position provides biopsychosocial assessments/interventions, crisis assessment/interventions, care coordination, referral coordination, and advocacy for patients and their support system(s) as a member of the interdisciplinary team for Lower Columbia Hospice.
This position oversees the bereavement program to support loved ones of hospice patients as well as providing grief support to the community through education and support groups, often in partnership with trained volunteers. This position is also supported by connection to the larger group of Social Worker and RN Care Coordinators in the Care Management department of Columbia Memorial Hospital.
Job Requirements
• Knowledge of/Skill/Ability to:
• Maintain the standards of Social Work excellence in accordance with principles set forth by the State Health Department, Columbia Memorial Hospital, and agency policies and procedures.
• Perform short-term crisis intervention and individual or family counseling when indicated.
• Strong time management and organizational skills
• Communicate and collaborate effectively with patients, families and multidisciplinary teams.
• Demonstrate understanding of diverse backgrounds and emotional intelligence, with the ability to provide compassionate, culturally responsive support. Proficiency in conducting cognitive and behavioral assessments, as well as delivering interventions to individuals, families, couples, and groups facing behavioral health and/or substance use challenges in a health care or home hospice setting.
License/Certification
• Must hold an Oregon Clinical Social Work license.
• If CSWA, Oregon Clinical Social Work Associate licensure required, or ability to obtain within 120 days of hire. Must provide a copy of your Masters in Social Work (MSW) degree if you do not have your CSWA.
• Ability to obtain LCSW within 3 years of start of full-time employment; for employment status outside of full time, a mutually agreed upon individualized timeline to obtain LCSW will be determined.
• If LCSW, Oregon Licensed Clinical Social Work required, or ability to transfer licensure from another state within 60 days of hire.
• Ability to maintain credentialing and appropriate documentation for insurance companies if needed.
• Valid driver's license with current vehicle insurance coverage and safe driving record is required.
• Advanced Certified Hospice & Palliative Social Worker (ACHP-SW) certification recommended.
Education and/or Experience
• Masters in social work from CSWE accredited school is required.
• Minimum of one-year post-Masters experience in medical social work preferred.
Physical Demands and Work Environment Conditions / ADA Compliance
Often work alone; travel to patient's home and other health care institutions-must drive or provide own transportation; normal or corrected vision is required-normal or corrected hearing is necessary; physical and emotional stress of dealing with death and dying. Position involves standing, walking, stooping, and occasional carrying and lifting of lightweight materials (up to 40 pounds). May be required to sit for extended periods of time. Requires visual acuity and depth perception to recognize words and numbers; and hand and finger dexterity and hand/eye coordination to use a computer keyboard and common office equipment.
