Position Description:

The Global Leasing program provides targeted assistance to Trueblood eligible individuals with complex behavioral health needs to obtain and maintain stable housing, which will ultimately address the interrelation of behavioral health concerns and housing stability. To achieve this work, the Housing Specialist will have two primary scopes of work: 1, home and community-based supports to eligible individuals- which includes coaching, advocacy, information and referral, linking and coordinating, and ongoing supports; and 2, identifying housing opportunities in the community by conducting outreach to private market, and scattered-site landlords in an effort to obtain global leasing opportunities. The Housing Specialist utilizes both of these areas of work to create a match between the eligible individual and an appropriate housing unit- and provide ongoing tenancy supports as needed (and for as long as the individual remains eligible) so that the individual can remain stably housed.


Essential Job Functions
:

  • Work closely with FHARPS team members to identify eligible Trueblood participants for Global Lease placements
  • Developing individualized housing plans based on the housing preferences of participants
  • Working with participants to develop short and long-term measurable goals
  • Providing service-planning support and participate in multi-disciplinary team and team planning meetings
  • Coordinating and linking participants to community support services i.e. primary care provider, behavioral health services, etc. Substance abuse treatment, including those supports designed to obtain and maintain independent living.
  • Providing entitlement assistance (including obtaining documentation, navigating and monitoring the application process, and coordinating with the entitlement agency)
  • Providing supports to assist participants in the development of independent living skills
  • Coordinating with participants to review, update and modify their housing support plan and crisis plan
  • Connect participant to resources that will assist in being a good tenant
  • Working with private market, scattered-site landlords to grow and foster opportunities for housing, specifically global lease opportunities, as well as transitional assistance to participants who wish to exit a global lease placement
  • Acting as a liaison with both FHARPS staff as well as landlords and property owners, to promote the success of individual participants and the global leasing program as a whole


Non-essential Job Functions
:

  • Supports agency mission, goals, and objectives.
  • Attend staff and planning meetings as scheduled.
  • Maintain communication and cooperative working relationships both within the agency and community at large, while maintaining a high degree of confidentiality.
  • Other duties as assigned.


Education, Licensing and Experience
:

  • Bachelor degree in Mental Health, Social Work, Psychology, or Human Services preferred but not required.
  • Approximately 2 years of experience in the social service field, within a supportive housing related program with mental illness/chemical dependency preferred.
