About the Role:
As a Social Services Assistant at Columbine Heath Services, you will provide services to residents and families who are faced with social, emotional and medical stressors. Serves as a resident rights advocate in all manner of speaking.
Join our team as a Rehab Unit Support Specialist! In this role, you will:
- Assist with admissions and discharges, advocating for residents and their families.
- Collaborate with Social Workers, facility departments, and community resources.
- Help residents exercise and protect their rights, creating individualized care plans.
- Provide resources for discharge and post-discharge planning.
- Ensure timely and accurate documentation and assessments.
- Coordinate and manage ancillary services, resident council, and neighborhood meetings.
- Schedule and participate in Care Conferences and interdisciplinary meetings.
- Support staff in managing social and emotional factors, crisis intervention, and audits.
- Adhere to facility policies, including HIPAA requirements, and act as a mandated reporter.
- Perform additional administrative and social services duties as needed. Be a part of a compassionate team dedicated to enhancing the lives of our residents! All other duties as assigned.
What We're Looking For:
Experience and Education:
- An associate's degree in human services preferred.
- Experience in healthcare preferred.
- Experience working with the geriatric population preferred.

Our Total Rewards package includes a salary range of between $17 - $19/hr. DOE
