Job Description:

ComForCare Home Care is a leading provider of in-home care services, dedicated to helping clients live independently and continue doing what they love in the comfort of their own homes. As a Community Relationship Manager at ComForCare, you will play a pivotal role in fostering and strengthening relationships within the community to drive new business. You will develop referral pipelines, build strategic partnerships, and represent the company in a variety of community-facing initiatives. This role requires a deep commitment to our mission of delivering exceptional care, while also ensuring operational efficiency, regulatory compliance, and confidentiality.

What We Are Looking For

  • High school diploma or G.E.D. certificate
  • Minimum of one (1) year of experience in community relations, marketing, or a related field
  • Self-driven, disciplined, and determined to independently drive results
  • Experience in marketing and relationship-building in the healthcare industry is preferred but not required
  • Strong communication skills to effectively engage with referral sources, clients, families, and internal teams
  • Knowledge of social media, SEO, and digital marketing strategies is a plus

What You Will Be Doing

  • Establish, implement, and manage community outreach initiatives to build brand awareness and generate referrals
  • Develop and negotiate contracts with managed care companies, government agencies, and other payers to ensure optimal service delivery and reimbursement
  • Ensure that client service requests are met according to individual care plans and client preferences
  • Implement workflow systems to optimize business operations and ensure compliance with agency policies and regulations
  • Participate in quality improvement activities and attend required meetings and training programs
  • Cultivate strong relationships with referral sources such as social workers, case managers, discharge planners, hospitals, senior living communities, rehab centers, and other healthcare professionals
  • Conduct regular visits to potential and existing referral sources to promote ComForCare’s services and encourage referrals
  • Educate potential clients and families about the benefits of home care services, aiming to increase client acquisition
  • Conduct group and one-on-one presentations on various topics to educate referral sources and the community
  • Be available during non-traditional hours as needed to manage and grow the business
  • Perform additional duties as assigned

Compensation: $21.00 - $25.00 per hour
