Position : Case Manager, RN
Schedule : Full-time, Monday to Friday, and / or on-call
Hourly range : $40-$55 / hour. DOE
Reports to : Executive Director of Patient Care

Responsibilities :
- Patient Care and Assessment : Monitor patients' conditions, assess their needs, and provide the best possible care.
- Symptom Management : Observe and interpret patients' symptoms, communicating to the Executive Director of Patient Care and updating the EMR system.
- Collaboration : Work alongside the entire Interdisciplinary team creating a positive experience of comfort to the patient and family.
- Procedures and Documentation : Perform routine procedures (e.g., blood pressure measurements, administering injections) and maintain accurate patient records.
- Medication Management : Monitor patients' medication needs and convey insights to the Medical Director and Executive Director of Patient Care.
- Supportive Environment : Foster a supportive and compassionate environment for patients and their families.
- Professional Development : Expand knowledge by attending educational workshops, conferences, and online training.
