Job Description/Summary: Rehabilitation Coordinator's work directly with individuals’ clients to assist with behavioral management, daily living skills, community support and other mental health and rehabilitative services as outlined in their assessment and treatment plans. Rehabilitation Coordinator's assists with the development of rehabilitation plans, and then provides direct psychiatric rehabilitation services and skills training to individuals recovering from mental illness and in recovery. The Rehabilitation Coordinator works with clients, individually and in group settings to assist clients in choosing and obtaining personal goals and developing the skills necessary for independence and success.
Job Duties & Responsibilities:
- Review initial and ongoing assessments to guide all interventions with assigned clients
- Complete monthly progress notes
- Participate in Treatment team meetings and other service coordination activities
- Communicate with the Rehabilitation Specialist/Counselor to ensure continuum of service
- Provide program coverage in all areas and in all aspects
- Provide client interventions in social skills training to improve communication skills and facilitate appropriate interpersonal behavior
- Problem solving, support, supervision of clients related to program and social activities
- Assist clients in gaining and utilizing skills necessary to participate in community reintegration– educational groups, behavior control, social skills, communication skills, ADLs support, successful interpersonal behavior, and coping skills
- Observe clients, provide support, positive reinforcement
- Implement interventions derived from the rehabilitation/treatment plan, reinforce client selected coping skills
- Teach and model appropriate social skills
- Participate in scheduled meetings to discuss client progress
- Involvement in the prevention of escalation, crisis intervention, decompensation and all necessary documentation following such events
- Maintain accurate and detailed records
- Follow program documentation policies in regard to documentation
- Track data as needed/assigned
- Initiate contact with outside agencies, visitors, and detailing relevant information from conversations
- Review and request feedback to maintain best practices and standards
- Maintain open lines of communication and uphold strong ethical and professional guidelines to ensure client care is superior
- Quality is measured through several areas:
- Evaluation of progress made by clients and communication related to their strengths and needs
- Quality and efficiency of the groups/activities in which the direct service worker oversees including completion of work timely and effectively
- Accuracy and timeliness of documentation
- Ability to provide support to clients and staff with little guidance necessary from leads
- Evaluation of documentation including behavior support notes, incident reports, and other necessary documentation related to client care
- Timeliness and attendance to shifts
- Expectation of adherence to all Title IX regulations, as well as program policy and procedures, including upholding this same expectation for those working on given shift
- Monitor the safety and security of the treatment environment and take necessary action for client safety
- Report changes in client physical conditions/behaviors
- Ensure resolution of client complaints when unresolved at lower levels

Pay: $15.00 - $18.00 per hour
