Dignity Health’s Pacific Central Coast Health Centers (PHC) is a non-profit Community clinic organization comprised of more than 40 health centers located primarily from Ventura to Templeton spanning the coast. Our experienced physicians and advanced practice providers offer a wide range of primary and specialty care services. Responsibilities
In keeping with the Pacific Central Coast Health Centers philosophy the CPSP (Comprehensive Perinatal Services Program) Educator manages health education assessment and reassessments interventions referrals and follow-up care for clinic patients with CPSP Case Managers and Providers.
Qualifications
Minimum Requirements:
- Minimum two (2) years perinatal case experience management; or equivalent combination of education and experience.
- Knowledge of perinatal services and Comprehensive Perinatal Services Program (CPSP) guidelines
- Good verbal and written communication skills.
- Good interpersonal and organizational skills.
- Works effectively with a variety of diverse populations.
- Can work with a team to plan develop and implement programs and services to promote community health education and health promotion thus improving the health of the individual patient and the community.
- Flexible; handles several projects concurrently.
- Strong attention to detail.
- Tactful sensitive and possesses knowledge of cultural differences when working with clients.
- General office experience and basic computer skills.
Preferred Qualifications:
- Bachelors degree in Social Services nursing or other related health field
- Bilingual English/Spanish preferred.
