A Community Health Nurse 1 provides professional nursing duties to support and maintain the physical and psychosocial health of DHS/AAA consumers. A Community Health Nurse 1 completes level of care assessments using the functional eligibility determination tool on consumers in their homes, hospitals, and other facilities.  After reviewing the Needs Assessment Tool, you will provide consultation for Care Management Caseworkers concerning the physical, medical, and psychological functioning needs of consumers. You will have the opportunity to serve as a resource, educating caseworkers regarding medical conditions, level of care, and care plans to promote optimum care for consumers. This position Identifies continuing educational needs of caseworkers, develops materials and tools for completing assessments and care plans according to PA Department of Aging.

Other duties to include but not limited to:

  • Consulting with partner agencies to evaluate quality assurance measures, compliance with regulations and program policies in care planning
  • Providing consultation and review of care plans for appropriateness of services and cost effectiveness.
  • Completing quality assurance by doing monthly monitoring for reviews of correct level of care and for complete documentation on (NAT's) and care plan worksheets
  • Providing guidance in resolving care planning problems related to medical/nursing or home health as needed


Work Schedule and Additional Information:

  • Full-time employment.
  • Work hours are 8:30 AM to 4:30 PM, Monday - Friday, with a 60-minute lunch.

QUALIFICATIONS 

Minimum Experience and Training Requirements:

  • A bachelor’s degree in nursing from a baccalaureate program accredited by the National League for Nursing: or
  • Any equivalent combination of experience and training.
  • Applicants will be considered to have met the educational requirements once they are within 3 months of graduating with a qualifying degree/credit.

Necessary Special Requirement: 

  • Possession of a current license to practice as a Registered Nurse issued by the Pennsylvania State Board of Nursing; or possession of a non-renewable temporary practice permit issued by the Pennsylvania State Board of Nursing.

Condition of Employment: 

  • Employees possessing nonrenewable temporary practice permits must obtain licensure as a Registered Nurse within the one (1) year period as defined by the Pennsylvania State Board of Nursing.
