Parent Navigator
POSITION SUMMARY: The Parent Navigator is responsible for engaging with families and supporting them in their goals. They will advocate for them with local systems and help them access community resources.
Accountable and responsible for the following results:
- Work collaboratively with the Family Support Specialist, family and involved supports.
- Travel the Greater New Haven service area to assist parents and families access community resources.
- Partner with the family and other supports to develop a strength-based, family-driven plan of care and ensure linkage to community resources.
- Participate in child and family team meetings.
- Engage families in program services.
- Advocate on behalf of children and families in various service settings.
- Educate, empower and mentor families to access supports in their communities and to take the lead in their own plan of care
- Document all contacts with families/supports in electronic health record and Provider Information Exchange (PIE) database.
- Perform any other department or agency related duties or special projects as directed by supervisor.
- Required to use own vehicle on company business and to transport clients, as needed.
- Regular and consistent attendance is an essential function of this position.

Qualifications
- High school diploma with at least three years’ experience working with families.
- Bi-lingual Spanish-English preferred.
- Knowledge of community resources and services.
- Must be available to work evenings and weekends to meet with families as needed.
- Ability to work independently and as part of a team
- Sensitivity to diverse cultures and language competencies.
- Strong communication and documentation skills.
- Must be proficient in Microsoft 365. Must be able to navigate through electronic health records system (EHR).
- Ability to prioritize and organize schedules/demands.
- Must have and maintain a valid driver’s license. Must maintain an acceptable driving record and be insurable at a reasonable rate under the Organization’s auto insurance plan.
Salary : $19 - $27
