The role!
Position : Supported-Living Assistant Program Coordinator
Location : Wakefield, MA
Pay rate : $24.50 per hour
Hours : 40 hours per week, days and hours flexible depending on needs of Individuals and Program

Summary & Responsibilities
The Assistant Program Coordinator responsibilities include management of the individual records, including the coordination of required weekly reports from Life Skills Mentors and the documentation of the individuals ISP and the Mentor Support Plan. The Assistant Program Coordinator will provide and / or coordinate daily living support to adults with developmental disabilities to help them live as independently as possible, using the home and community as the classroom. Manage the 118 Nahant St and the individuals residing there. This is a leadership role.

Qualifications
Physical Demands :
The employee must occasionally lift 30 pounds alone, or up to 50 pounds with assistance of another person or adaptive equipment while performing the duties of the job. The employee is regularly required to talk or hear. The employee frequently is required to stand, use hand to feel, reach with hands, arms, climb or balance and stoop. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus.
Work Environment :
While performing the duties of this job the employee must drive to various appointments and spend time in various homes of individuals supported. The noise level in the work environment is usually moderate.
