What are the responsibilities and job description for the Cal-AIM Housing Case Manager position at Community Action Partnership of Kern?
SUMMARY: Under the supervision of the Program Supervisor the Cal-AIM Housing Case Manager works with community service providers to build relationships with the community, provide advocacy, and link to benefits and housing. The primary goal is to identify unsheltered homeless and other individuals through community referrals from homeless service providers and social service agencies. In identifying highly vulnerable homeless, unsheltered, and other individuals, the Cal-AIM Housing Case Manager will support Medi-Cal members experiencing homelessness or those at risk of homelessness, helping them navigate housing resources through Community Supports services provided by Medi-Cal Managed Care Plans (MCPs). The role is vital in addressing health-related social needs to promote healthier living and prevent more costly healthcare interventions.
