JOB SUMMARY
Position Title : Parents as Teachers (PAT) Home Visitor - County
Program : Parents as Teachers
Classification : Non-Exempt / Full Time
DUTIES AND RESPONSIBITIES
Home Visitation :
- Complete home visitation frequency in accordance with PAT Quality Standards, as directed by supervisor.
- Recruit families for Parents as Teachers and complete all necessary enrollment forms
Developmental Screenings and Family Assessments :
Assess children through required developmental and health screenings and performance measure assessmentsMonitor age-appropriate milestones and support healthy child developmentComplete family assessments to support the goal setting processProvide appropriate resource connectionsData Collection :
Plan, implement, and record all home visitation and required documentation according to PAT Quality StandardsMaintain up-to-date, thorough and accurate home visiting records, including regular data entry and evaluations in the appropriate database(s)Document caregiver goals to support family well-being; monitor progress on goals regularlyCommunity Collaboration :
Provide family support through community resources and group connectionsMake appropriate referrals to community resources and advocate on behalf of families when appropriateWork collaboratively to plan, implement, and encourage family attendance during family engagement eventsUse strong interpersonal skills when interacting with families and local partners, and demonstrate flexibility when changes ariseFacilitate communication and collaboration between community partners to improve kindergarten readiness and overall family wellnessParticipate in First 10 meetings and community partner meetings, as appropriateParticipate in all required team meetingsDevelop an understanding of and a commitment to the mission of CAP, as well as the mission community partners, as appropriateProfessionalism :
Work independently and have a strong sense of responsibility in carrying out the required job duties with a minimum amount of daily supervision.Meet regularly with the supervisor for reflective supervision, and as appropriateIdentify professional development opportunities to continually increase knowledge of kindergarten readiness skills to share with familiesDemonstrate CAP values during interactions with families, team members, supervisors, and community partners.Maintain confidentiality regarding program familiesPerform other duties as assignedEDUCATION / EXPERIENCE REQUIREMENTS
Preferred :
Associates degree or higher in Early Childhood, Human Services or related field and two years combined work experience related to families in education, social services, health services, or related outreach workRequired :
High School Diploma or GED and a minimum of 2 years' previous supervised work experience with young children and / or familiesProvide and maintain satisfactory clearances : Pennsylvania Child Abuse History, Pennsylvania State Criminal Record Check, and FBI Criminal History ReportAbility to complete the Parents as Teachers Certification TrainingAbility to work non-traditional hours and / or attend out-of-town trainingAbility to communicate with people of various socio-economic backgroundsPlease see attached for full job descriptionWe Value our Team by uniting our employees with the mission.
