The Bilingual Family Case Manager, Better Birth Outcomes (BBO) is responsible for the following:

Key Skills: Proven ability to manage documentation, strong organizational skills and practices, comfort and effectiveness visiting families in the community and in their home setting, demonstrated ability to establish productive rapport with families, and the ability to appropriately handle difficult topics.  Successfully conduct outreach in the community and schedule interview appointments in the office and community settings. 

Desirable: Knowledge of Illinois Department of Human Services (DHS), Early Intervention, community supports and general services as well as current standards/recommendations for prenatal care and infant care & development, early intervention, community supports and general services.

  RESPONSIBILITIES

  • Preliminary review of participant information received and successfully completes intake to enroll clients in BBO/FCM Program as directed by supervisor.
  • Ensures that each participant has a Primary Care Provider and if not, link them with the appropriate medical services.
  • Identifies service providers for BBO/FCM's clients and makes appropriate referrals to link clients with the community-based services needed.
  • Develops/maintains and updates comprehensive individual care plans.
  • Conducts outreach at service sites and makes visits to participants' homes as necessary and/or required by program specifications.
  • Completes internal time and activity in Cornerstone for submission to funding sources.
  • Follows guidelines of case confidentiality according to agency standards.
  • Maintains case records on his/her caseload and provides information on cases as necessary or required.
  • Remains reachable by phone until 5:00 pm during the week.
  • Responsible for returning messages/client calls as assigned or for finding a backup if scheduling activities to be out of the office.
  • Participates in agency and BBO/FCM program staff meetings.
  • May be asked to attend meetings within Chicago area for the purpose of information exchange.
  • Upon assignment, participates in special projects.
  • Cooperates with peer staff and works as a team member in the negotiated sharing of common tasks associated with accomplishing common goals assigned.
  • Exercises diplomacy and professional demeanor when procuring or reviewing services for clients and demonstrates an understanding of the organizational structure of human service agencies.
  • Adheres to any Contract Agreements between CAU and other service providers. (Transfer policy procedures, WIC outreach, etc.).
  • Makes presentations of CAU's services to groups with Program Director as requested.
  • Participates in CAU case management staffing as well as staffing with identified service providers related to a specific client as assigned.
  • Participates in program evaluation and needs assessments as requested/assigned.
  • Exercises professional ability to work independently on a day-to-day basis.
  • Develops collegial supports within the community.
  • Other duties as assigned by supervisor.

WORK ENVIRONMENT

This job operates in a professional office environment and in the community service area. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.  The employee may be exposed to a variety of conditions at client/family homes and client selected meeting sites, consumer and family homes. The noise level in the work environment and job sites can be loud. This position requires exposure to outside weather conditions.

While performing the duties of this job, the employee is regularly required to talk or hear. The employee is frequently required to sit; stand; walk; use hands to finger, handle or feel; and reach with hands and arms.  Employee must be proficient in computer skills and abilities.

POSITION TYPE/EXPECTED HOURS OF WORK

This is a full-time position; 40 hours of work, flexible based on client’s needs and program/outreach schedule as assigned by program director and/or supervisor. 

TRAVEL

Employee must have reliable transportation.  Travel within the CAU Service area is required.  No overnight travel required.

This position requires an individual who is capable of interfacing with staff of various agencies providing referral, medical, support, and generic services to eligible recipients. This position requires basic computer skills with a readiness to learn the Cornerstone: Information system.   A good knowledge of the Department of Human Services, prenatal care, Early Intervention, community support and generic services is preferred. In addition, a valid driver’s license, current insurance and a reliable car are needed.

QUALIFICATIONS

  • Minimum: Master’s Degree in Behavioral/Social Science, Master's Degree in Social Work,  Master's Degree in Counseling Psychology, Master's Degree in Human Services and Counseling, Master's degree in Public Health, Licensed Professional Counselor (LPC), Licensed Clinical Professional Counselor (LCPC), or a related field or Registered Nurse (RN).
  • NOTE: If you earned your degree from a college or university outside the United States, a copy of a Certified Translation of Degree must be provided.
  • Bilingual (Spanish/English) preferred.
  • Exposure and experience working with pregnant women, families, and children 0-6 years of age.
  • Experience with community health, maternal and child healthcare.
  • 1 year of experience in data entry and data processing.
  • Ability to establish good rapport with families and handle difficult topics.
  • Must be computer-literate.
  • Strong English language skills.
  • Excellent telephone, interpersonal and written communication ability.
  • Must provide own reliable transportation to travel to clients in the CAU geographical area as needed or required. 
    The CAU Family Case Management program geographical area covers Chicago (primarily north of Roosevelt) plus the West, Northwest and North suburbs of Cook County (primarily north of I-55).
