Your Role

As a Case Manager in the Better Birth Outcomes Program at Near North Health, you will empower high-risk pregnant women and their newborns by providing intensive case management and home visitation services. Your role will be pivotal in ensuring healthy deliveries, promoting postpartum care, and connecting participants to essential resources. Through individualized care plans, service coordination, and ongoing support, you’ll help improve maternal and child health outcomes. Your passion for community health and dedication to fostering strong, supportive relationships will make a lasting impact on the lives of the families you serve.

Qualifications 

• Education Requirements: One of the following is required:
  • Masters of Social Work
  • Masters of Arts
  • Masters of Public Health
  • Licensed Professional Counselor
  • Licensed Clinical Professional Counselor
  • Registered Nurse

• Years of Experience: 2 years of experience in community health or maternal & child health OR 1-year experience in child, family or community services. Good organization and problem-solving skills.

Compensation 

Compensation is determined based on each candidate’s experience, skills, and education, within the established range for the role. Candidates who meet the minimum requirements will typically start at the entry level of the range, with additional qualifications reflected in the final compensation package.
