Salary: $23.55/hr

POSITION OBJECTIVE: This part-time position provides case management for older youth and young adults in or previously in foster care that are eligible or potentially eligible to receive services and support through the youth services department. The Youth Services Specialist provides assessment, planning, linkage, coordination, monitoring, evaluation and advocacy to the young adult, support system, and landlord/caregiver. There are two positions open, one in Orange County and one in Osceola County.

ESSENTIAL FUNCTIONS:

  • Provide Independent Living Case Management for older youth in foster care in accordance with Florida Statutes, Florida Administrative Code, and DCFOP.
  • Develop, strengthen, and maintain a direct working relationship with the young adult, the young adult’s own support system, mental health professionals, and community agencies.
  • Provide guidance and support to case management agencies regarding Youth Services Programs, Requirements, and Services.
  • Promote and recruit Teen programs available to youth, caregivers, and case managers throughout the system of care.
  • Prepare all parties for the Youth Transition Meeting
  • Prepare court documents and attend court for young adults in Extended Foster Care.
  • Complete program applications and funding authorizations for youth receiving post 18 services and maintain at least monthly contact with the young adults to ensure the transition plan is being followed or updated as necessary.
  • Maintain open communication with the Youth Services Team and other community partners to connect youth and young adults that need assistance in specified areas.
  • Attend monthly meetings with Youth Services Team Members to collaborate and work as a team to support the needs of the teens and young adults served.
  • Participate in the Agency’s Quality Assurance and Improvement activities deployed throughout the organization to include contracted providers including technical assistance, training, and assistance in policy development as needed.
  • Assists in the collection, analysis, and reporting of program data to enhance the capacity of the agency to make timely and effective adjustments to the operation of the agency’s Child Welfare System of Care.
  • Participates in creation and annual reviews of Unit Policy and Procedure Manuals to ensure all policy and procedures reflect current state, federal and system of care requirements.
  • Participates in the agency strategic plan & quality improvement processes
  • Ability to safely & successfully perform essential job functions consistent with the ADA, FMLA, & other federal, state, & local standards, including meeting qualitative & quantitative productivity standards.
  • Ability to maintain reasonably regular, punctual attendance consistent with ADA, FMLA & other federal, state, & local standards
  • Ability to comply with all personnel policies & procedures

QUALIFICATIONS:

REQUIRED:
  • Bachelor’s degree in social work or related area of study
    • Related degrees include human services, criminology, juvenile justice, psychology, sociology, counseling, special education, education, human development, child development, family development, marriage and family therapy, and nursing
  • 2 years’ experience in child welfare services.
  • State of Florida Child Welfare Certification (CWCM)

PREFERRED:
  • Master’s Degree from an accredited college or university.

  • Ability to travel
SPECIAL CONDITIONS:
  • Thorough Background Screening upon hire including FBI, FDLE, Local Law check, Employment Reference check, DMV check, Criminal Records check, Social Security Trace & Drug Screening. FDLE check will be run at least every 5 years.
  • On-call assignments- Yes
  • Travel- local: 10% per week, long-distance: 1% per month
  • Attend at least 15 hours of in-service training annually
