Summary

We are seeking a Client Services Coordinator to join our team. This role is vital in ensuring our clients receive the highest level of care and support. The Client Service Coordinator is responsible for providing case management for the HCP and HOPWA program, and oversees the Food and Hygiene Pantry in Ukiah. This position assists the Program Director in planning fundraising events and supports the case managers with the daily functions within the office.

Responsibilities

  • Assess client charts and case managers’ paperwork for accuracy and content as well as implementing program policies and procedures to assure on-going compliance of requirements established by the Office of AIDS.
  • Assist Program Director with data management, contract reports, and annual audits.
  • Process Case Managers’ client paperwork, complete quality assurance forms, and update work status.
  • Print and distribute progress notes for case managers to sign on the 10th of each month.
  • Update client information, including demographics, documentation, and log services into Aries within 14 days of provided services.
  • Assist clients with applying for HOPWA services for emergency housing and utility assistance as guidelines and supervisor direct.
  • Accurately interpret and dispense financial assistance guidelines for HOPWA and HIV Care Program as directed by funding sources guidelines as well as cost avoidance criteria.
  • Assist HIV Care Program clients with housing and financial management plans to increase their level of independence and stable housing, decrease the need for emergency services.
  • File paperwork in client charts on a weekly basis.
  • Assist clients with paperwork, make referrals, and support clients using a Harm Reduction Model.
  • Distribute and log transportation and nutritional vouchers from the 15th to the end of the month.
  • Update client contact information in appropriate databases and chart.
  • Assist Program Director in planning and executing the Annual AIDS Walk and other fundraising activities.
  • Oversee Food Pantry including the maintenance and cleanliness of the space. Call clients for pantry reminders and other information, as needed.
  • Keep Food Pantry adequately stocked and organized within the set standards. Check temperatures in refrigerators and freezers weekly and enter into log.
  • Data Entry into ARIES for all clients served and services offered through Early Intervention Services. Enter case management units of service into ARIES for HCP, WAIVER, and HOPWA.
  • Oversee purchases for office and clients.

Requirements

  • Two to five years of administrative work and/or experience working in human services with diverse populations and social needs.
  • Associates Degree in psychology or social work, plus two years experience in the field of health and human services.
  • Strong computer skills and interpersonal skills.
  • Must possess Food Handler's card within 30 days of employment.

Work Environment

The workspace for the Client Service Coordinator is located in a heavy traffic area. Frequent interruptions from both fellow staff members and clients by phone or in person is expected. Upkeep and maintenance of the Food Pantry make this a physically active and demanding position. Frequent local travel to pick up inventory for the pantry, office errands, and transporting items to client homes are to be expected. The Client Service Coordinator is required to take occasional trips outside the County to attend training, department meetings, and agency events.

Additional Requirements

  • All employees must pass State and Federal Live Scan background check before the start of employment.
  • Must possess Food Handler's card within 30 days of employment.
  • All employees of Community Care must carry the minimum bodily injury liability insurance on vehicles used for work.

Pay: $19.41 - $21.21 per hour
