Community Care of the Lower Cape Fear is hiring a Care Navigator for our Human Services Organization, based in our Wilmington, NC office.
Job Purpose
The HSO Care Navigator will facilitate the Fruit and Vegetable Prescription service as well as assist with housing supportive services as part of CCLCF’s Human Service Organization (HSO) within the Healthy Opportunities Pilot (HOP). The Care Navigator has an integral role in the development and implementation of navigation activities as outlined in the HOP service descriptions to assist individuals to obtain needed resources regarding housing insecurity and other Social Determinants of Health (SDOH). The HSO Care Navigator works collaboratively with the Housing Navigation Care Managers and under the supervision of the Team Lead.
Essential Duties and Responsibilities
· Conduct outreach, engagement, and enrollment of referred services.
· Complete thorough and timely documentation in a designated HIPAA- compliant documentation system to assist with time-sensitive needs.
· Provide effective and appropriate care navigation at all phases of the process from receiving referral to outreach and service delivery through case closure.
· Coordinate housing supportive services including but not limited to housing move-in services, utility assistance, home goods, payments for security deposits and rent within the CCLCF HSO.
· Assist members with completing housing applications, obtaining necessary documentation and paperwork and other materials needed for housing services, and creating housing sustainability plans.
· Collaborate with Housing Navigation Care Managers on referrals and additional services that members may need.
· Provide clear communication with members via mail and telephone regarding the Fruit and Vegetable Prescription service.
· Administer and coordinate Fruit and Vegetable prescription vouchers using electronic platform including reloading, monitoring, and reporting on the vouchers.
· Work closely with the finance team to coordinate invoicing and billing.
· Complete thorough and timely documentation of all interactions. in designated HIPAA- compliant documentation system.
· Encourage and promote an environment that is person-centered, inclusive, and culturally sensitive to assist members in meeting their individual goals.
· Collaborate with HOP Network Lead, Medicaid Prepaid Health Plans (PHPs), community agencies, coalitions, landlords/complexes, other HSOs, internal and external care managers, and more.
· Perform other duties as assigned and as outlined in the HN HOP service description.
Qualifications
· Associate’s degree in related area or equivalent work experience that provides the knowledge, skills and abilities to perform the job.
· Valid NC Driver’s license with current auto liability insurance
· Excellent communication skills – oral and written
· Ability to learn and demonstrate proficiency in Motivational Interviewing techniques
· Proven attention to detail, customer focus and ability to flex priorities in a fast-paced environment
· Strong organizational and time management skills
· Sensitivity to diversity of cultures, language barriers, health literacy and educational levels
· Experience with Microsoft Office Suite, desktop publishing software, and virtual meeting platforms
· Must have received or be willing to receive the influenza vaccination by date of hire (during flu season only) and annually thereafter to be considered. Proof of vaccination is required.
Preferred Qualifications:

· Knowledge of medical terminology
Physical Requirements
- May be required to sit for extended periods of time, lift objects up to 20 pounds on an occasional basis and do repetitive tasks such as typing on a regular basis.
- Multiple contacts are required with various members, providers, multi-payer systems and community partners to ensure coordination of services, therefore, exposure to general office and household conditions, as well as communicable disease, could occur.
