PRIMARY FUNCTION:
The APRN/NP is responsible for providing outpatient diagnostic, treatment, and preventive healthcare services; management of chronic illness; utilizing appropriate referrals to specialists; health education to patients. The schedule aligns with the 10-month schedule for the State of Hawaii Department of Education as published for "in-session" scheduled days and times. The APRN/NP-SBHC will be assigned to work in a Community Clinic of Maui, Inc. health center on days when the school is not in session.

In the SBHC setting, the APRN/NP works with an MA, BH CHW, and LCSW.
The APRN/NP also plays an essential part in the clinic attaining the goals that administration has set forth for reaching Patient Centered Medical Home recognition, UDS standards and third-party incentive programs. The APRN/NP will do this in a team-based care model by functioning in the team as the primary care facilitator.
ESSENTIAL RESPONSIBILITIES
- Provides comprehensive healthcare services to all patients, including but not limited to, well-child care, acute care, and chronic care.
- Provides patient and family education to promote wellness, to prevent health problems, and to maintain current health status as well as intervene in acute or chronic illness states.
- Coordinates client care with other community professionals and organizations for optimal care.
- Works collaboratively with supervising pediatrician, programs director, chief operating officer, and other health center professionals.
- Works collaboratively with the school to determine student needs and align clinic care and offerings accordingly.
- Participates in chart audits as specified in the quality improvement program.
- Provides input into decision-making process by attending provider and clinic staff meetings as scheduled.
- Assists in developing goals and objectives for the program including quality improvement.
- Participates in problem solving of issues and concerns affecting the SBHC program.
Quality and Safety:
- Participates in self-education needed to perform case management and patient care.
- Participates in meeting the quality clinical measures set by the organization by reviewing preventive care needed and protocol tests, as needed.
- Participates in quality initiatives as directed to fulfill the organizations grant obligations and insurance requirements.
- Practices within the Clinical Protocols and Guidelines and Policies and Procedures of the organization.
- Participates in Emergency Preparedness Plan.
Other:
- Other duties, as assigned, and within the scope of licensure.
- Maintains current continuing educational hours, certifications, and licensure.
- Participates in coverage as needed for patients and other assignments, including school events.
- Provides comprehensive healthcare services to all community patients, as assigned, and with planned expansion of services in the assigned school community.
Requirements
QUALIFICATIONS:

Education:
- Graduate of an accredited school of Licensed Professional Nursing, MSN-APRN with pediatric experience is highly preferred.
- Licensed in the State of Hawaii as a MSN-APRN or FNP
Experience:
- Two (2) or more years’ experience as a MSN-APRN, Pediatric NP, FNP preferred; experience in a school-based work environment is desirable but not required.
- Experience or training in case management preferred.
- Prior experience and familiarity with the population and the needs of the community.
- Must understand the fiscal and programmatic administration of a federally funded Community Health Center.
- Must understand the needs of the consumer and understand the complex management of a diverse, community-based health care program.
Skills/Competencies:
- Excellent verbal and written communication skills.
- Ability to plan and organize small and large scope projects.
- Ability to make accurate assessments and prioritize work to be done.
- Must have ability to logically analyze and rationalize issues of a complex nature and make decisions with large ramifications.
- Must possess strong leadership skills and ability to solve problems.
- Computer literate in word processing and spreadsheets. Knowledge/skills of basic computer operations, i.e. word processing, data base, including using Microsoft Word and Excel to create reports and graphs.
- Provide excellent customer services to our clients including demonstration of the organizational value that patient needs are the first and central priority and the belief that patients come first.
- Willingness to assist co-workers.

- DEA Registration Certification-federal
- NED-Controlled Substance Registration Certification-State of Hawaii
- TB Clearance within the past 12 months
