What are the responsibilities and job description for the Health Coach position at Community Clinic?
Community Clinic is seeking Health Coaches to work in our medical clinics throughout Northwest Arkansas. The Health Coach is a member of a multidisciplinary team, serving as the link between patients and their healthcare providers, social services and community resources. Key responsibilities include conducting patient needs assessments, providing health coaching to a panel of patients with chronic conditions, as well as facilitating case management and coordination of services.
An associate’s degree or higher is preferred. Bilingual English/Spanish is a plus. Upon hire, the selected candidate will be required to successfully complete an employer-sponsored health coach training and certification course. These are full-time positions, M-F.
