Job Summary
Community Clinic is seeking Health Coaches for to work in Rogers, Springdale, Centerton, and Siloam Springs. A Health Coach is a member of a multidisciplinary team, serving as the link between patients and their healthcare providers, social services and needed community resources. The Health Coach uses evidence-based tools to help patients make and maintain meaningful changes for improved health and well-being.
Responsibilities
- Conducts interviews with patients to assess needs, current health status, desired changes/goals, and barriers.
- Provides health coaching to promote wellness and chronic disease management, using individualized plans of care.
- Facilitates communication and coordinates services between patients and the multidisciplinary health care team.
- Performs case management duties by connecting patients to appropriate community resources.
- Networks with community agencies to build relationships and stay current on service offerings.
Qualifications
- Strong interpersonal skills and the ability to work effectively with people of all backgrounds.
- Associate’s degree or higher is preferred.
- Case management, community health worker, health coaching, or related experience is a plus.
- Upon hire, required to successfully complete an employer-sponsored health coach training and certification course.
- Bilingual Spanish/English preferred
