What are the responsibilities and job description for the Case Manager (QIDP) - Developmental Disability Services position at Community Connections, Inc.?
Community Connections, Inc. is Idaho's premier agency serving adults with developmental disabilities, and we are seeking an experienced, motivated and caring Case Manager / QIDP to work out of our Post Falls location's Supported Living program. The caseloads are small, allowing professionals to truly impact the lives of those they serve!
Case Managers are responsible for supporting participant success while ensuring paperwork obligations are met, including, Behavior Management program writing, team collaboration, participant satisfaction and quality assurance oversight. Our goal is to ensure we work together as a team to assist participants in achieving maximum independence in our 24-hour programs.
Case Managers must have experience working with individuals with developmental disabilities, including running Behavior Modification programming, and a four-year bachelor's degree in Psychology, Special Education, Sociology, Criminal Justice, Social Work, Health or another human service field; the experience and degree criteria is REQUIRED in order to apply.
