Description

Greater Nashua Mental Health is looking for a Community Support Specialist to join our Deaf Services team! The Community Support Specialist meets with Deaf and Hard of Hearing individuals statewide in their natural environments, providing home-based community services to support their recovery from mental health conditions. Assists them during crises, educates on various recovery-related topics, and helps remove barriers to improving functioning, facilitating optimal recovery.

Duties & Responsibilities :

  • Engages clients in targeted interventions, including evidenced-based interventions (i.e., Motivational Interviewing), aimed at supporting improved functioning in the community.
  • The standard KPI for this position is to provide direct client care to individuals 42% (24 hours each week). There may be adjustment made for approved additional duties.
  • Conducts outreach for clients within assigned caseload when disengaged in treatment, per the missed client appointment policy.
  • Completes all documentation for each service delivered, outreach attempts, and team discussions within 7 business days (with first day being date of service / event). Strives to complete documentation collaboratively with clients so notes are completed on the same day of service.
  • Attends mandatory weekly team meetings for primary caseload assignment, in order to discuss and document pertinent client care discussions, as part of an integrated team approach.
  • Enters daily schedules into the EMR.
  • Attends individual scheduled supervisions, weekly group supervisions, and required trainings as part of professional development and to enhance client care.
  • Completes and submits mileage / expense sheets within the month they are due.
  • Performs other duties or special projects as required or as assigned.

Requirements

Qualifications :

  • High School Diploma or GED equivalent required. Associate's Degree, in Human Services, Psychology, Social Work or other related field is preferred
  • Fluent in American Sign Language (ASL) and English
  • 1 year related work experience
  • Basic education on mental health conditions
  • Understanding of the components of recovery
  • Strong collaborating skills and an ability to work effectively both independently and as part of a team
  • Comfortable in a fast-paced environment with frequent deadlines; high energy, self-motivated and proactive
  • Excellent communication and interpersonal skills
  • Ability to multitask, and be flexible with regard to workload and assignments
  • Ability to exercise judgment
  • Knowledge of Microsoft Office products (Email, Word, EXCEL)
  • Must possess a valid driver's license and automobile with liability insurance in the amount required by agency policy
