Permanent Supportive Housing Case Manager:
This Case Management position works with chronically homeless adults in a permanent supportive housing environment. This position supports the mission of CCM by assisting with the development and implementation of community and support. The Case Manager will work with the residents, Housing Management and community partners to identify needs and coordinate resources that promote self-sufficiency, empowerment and a healthy community including, but not limited to, substance abuse referrals, mental health referrals, employment counseling, financial counseling and housing retention.
Key responsibilities may include:
- Client intake and assessment:
Conducting thorough assessments to identify individual needs, including housing, mental health, substance use, medical, financial, and employment situations.
- Individualized service plans:
Collaborating with clients to develop personalized goals and service plans tailored to their specific needs and aimed at achieving housing stability.
- Housing search and placement:
Assisting clients in finding suitable permanent housing options within the PSH program, including application assistance, landlord communication, and move-in support.
- Case management:
Providing ongoing support to clients through regular check-ins, monitoring their housing stability, addressing challenges, and connecting them to necessary community resources like healthcare, benefits, and employment services.
- Crisis intervention:
Responding to immediate crises that may threaten a client's housing stability, including providing emergency support and coordinating with relevant agencies.
- Collaboration with other providers:
Working closely with mental health professionals, substance abuse counselors, medical providers, and other community partners to coordinate care and ensure comprehensive service delivery.
- Advocacy and outreach:
Representing clients' needs to landlords, government agencies, and other stakeholders to advocate for housing stability and access to necessary services.
- Documentation and reporting:
Maintaining detailed client records, progress notes, and required reports to track outcomes and comply with program guidelines.
Required skills and qualifications:
- Master’s degree in social work preferred, or Bachelor degree human services, or a related field. With a minimum five years’ experience
- Experience working with individuals experiencing homelessness, preferably with complex needs such as mental illness or substance use
- Knowledge of community resources and service systems
- Strong case management skills, including assessment, goal setting, and crisis intervention
- Excellent communication and interpersonal skills to build rapport with clients and collaborate with various stakeholders
- Ability to work independently and as part of a team
- Proficiency in data entry and record keeping
- Experience working with diverse populations preferred
