What are the responsibilities and job description for the Direct Care Professional position at Community Counseling Center of Mercer County?
Minimum of $14.50 an hour!
POSITION: The Residential Program Worker supervises and assists the residents in all daily life skills. They report directly to the Residential Supervisor. They maintain the facility they are assigned to.
QUALIFICATIONS: High School Diploma with human services experience in a Residential setting. Possess excellent communication and interpersonal skills. Act 33 and 34 and FBI Clearances required. Must maintain a current driver's license and vehicle insurance.
Responsibilities:
- Arrive for work on time, i.e. at least 5 minute prior to the beginning of your shift.
- Provide coverage for any assigned site.
- Read daily progress notes from previous staff to be aware of recent occurrences in the home.
- Is aware of daily programming of the residents and assure that they do attend as scheduled.
- Have residents abide by House rules and to appropriately document any incidents.
- Correctly completes merit system paperwork daily.
- Complete daily progress notes on residents.
- Completes all other assigned paperwork daily or as required.
- Correctly monitors medication according to medication procedure. (See Policies and Procedure Manual).
- Initial medication sheets after each resident.
- Inform residents when medication refills are needed and to assist residents in procedure to refill medication.
- Count medications before being placed in medication cabinet and to record the count on proper form.
- Perform daily room checks before 10:00 A.M. each morning and to properly record score.
- Engage and encourage residents to participate in group discussions and activities.
- Complete individual training sessions with residents as instructed by Supervisor.
- Assist residents in achieving goals.
- Assist residents in daily cooking.
- Assist residents in daily assigned cleaning chores.
- Assist residents in laundry skills and all other daily living skills.
- Assist residents in all other life skills as required.
- Be available to talk to any resident as needed.
- Respect the residents and educate them in a professional, mature manner.
- Handle any immediate problems in the home, referring serious problems to appropriate staff.
- Use emergency procedures appropriately (See Policy and Procedure Manual).*
- To know the locations of all emergency shut off valves, and breaker boxes.
- Maintain open positive communication and cooperative attitude with other staff and supervisor.
- Provide transportation, as needed for any resident, to appointments.
- Attend all mandated CCC in-service training, maintain current CPR and First Aid certificate.
- Attend monthly staff meetings the last Wednesday of each month.
- Follow the programming schedule established for the site.
- Hold monthly fire drills, and ensure that all licensing regulations are upheld in the residents.
- If leaving the group home during the hours of 8:00 A.M. - 4:30 P.M., notifies appropriate personnel.
- Will follow through with present on-call procedure.
- Physical demands of this job require bending, stooping, climbing, reaching, grasping, finger dexterity for writing and data entry on keyboard, ability to hear, see, read and drive up to a 16 passenger van. Able to lift and carry up to 15 pounds.
- Maintains and upholds Community Counseling Center's policies, respects confidentiality, and exhibits a professional demeanor at all times while at work.
- Any violation of policies and procedures/job duties will be subject to disciplinary action.
- Performs other duties assigned.

- Participates in CCC CQI plan to ensure quality of service to CCC consumers.
