Intake Specialist
Job Description
Purpose:
Directs individuals interested in mental health treatment to the services which meet their needs. Ensures that thorough assessments are completed, and individuals have options for case management and group services while they wait for long-term treatment. In a culturally sensitive and appropriate manner, provides and coordinates these services to programs within the agency. Serves as advocate for C4’s mission, vision and brand.
Minimum Qualifications:
- Master’s Degree in Social Work/Psychology, an appropriate human service field, or relevant field preferred. Well-qualified Bachelor’s degree level candidates will be considered
- 1 year of assessment and case management experience strongly preferred
- Well-organized with a client-centered approach
- Excellent knowledge of MS Office Suite

Duties and Responsibilities:
- Manage referrals for mental health services
- Schedule in-person or video intake appointments for determination of services
- Conduct mental health assessment interviews, which include clinical interviews of adults, make written recommendations for ongoing services, determine preliminary DSM V diagnosis, and identify treatment goals and objectives
- Complete the Illinois Medicaid Comprehensive Assessment of Needs and Strengths (IM CANS) for every new client
- Administer screening and assessment tools based on client needs and program requirements
- Collaborate with a variety of clinical teams to ensure access to appropriate services
- Provide case coordination and linkages to community and internal resources to ensure individual’s needs are met
- Oversee service assignment and ensure a smooth transition to assigned clinician
- Communicate assignment status to clients
- Meet documenting, record-keeping, and direct service requirements of the position
- Attend regular supervision with designated program supervisor and attend team meetings when deemed necessary
- Collect, review and utilize data on services to inform program/department expansion
- Consult with assigned clinicians about clients
- Attend required trainings
- Facilitate skill-based groups for families
