Purpose:
The Intake Care Coordinator will be responsible for facilitation of C4 service delivery between internal and external programs, partnerships, and stakeholders for the purpose of connecting prospective clients to desired service(s). Establishing and maintaining fluid and transparent relationships between internal and external programs, partnerships, and stakeholders. The ideal candidate will possess high level networking ability, be a skilled client advocate, well organized and able to problem solve.
Location and Work Conditions:
- Normal office environment including movement from desk to other work areas in a multi-floor suite and multi-building sites to carry out job duties
- Might also be required to provide supervisory consultations over the phone while off site or outside of normal business hours
- Required to drive to all C4 site locations as needed
Duties and Responsibilities
- Manage referrals for C4 service provision
- Communicate between staff, clients, and programs both internal and external
- Building and fostering rapport with internal and external partners
- Complete the Illinois Medicaid Comprehensive Assessment of Needs and Strengths (IM CANS) for clients on an as needed basis
- Meet documenting, record-keeping, and direct service requirements of the position
- Facilitate a seamless transition from client entry point to a desired service
- Monitor client services post placement
- Understand Mental Health Diagnosis Criteria as outlined in the DSM 5-TR
- Generate reports detailing care coordination efforts between internal and external partners
- Facilitate in-person and virtual meetings between internal and external programs
- Attend required trainings in-person and virtual
- Participate in regular supervision with assigned supervisor
- Other duties as assigned
Minimum Qualifications:
- Bachelor’s degree in social work or related field from an accredited university required.
- Master’s degree in social work, Psychology, Sociology, or an applicable human service field from an accredited university preferred.
- Minimum three (3) years’ experience in healthcare administration/management with experience in operations management, and/or finance preferred.
- Knowledge of federal, state, CMS, Medicaid and various licensing, certification and accreditation requirements.
- An equivalent combination of academic, clinical and operational experience may be considered
- Current, valid, and active driver’s license required.

- The ability to assess co-occurring disorders
- Understand the Social Determinants of Health relative to client population needs
- Awareness and utilization of Motivational Interviewing
- Familiarity of the American Society of Addiction Medicine (ASAM) criteria
- Understand and practice a client centered approach
- Familiar with Mental Health Rule 132
- Understanding of cultural awareness and humility
