The Connections Coordinator works as a multidisciplinary team member in providing screening/assessment of needs, crisis management, monitoring, advocacy, barrier elimination, education/training, coordination/linkage and empowerment/skill building services to short-term mental health customers. The Connections Coordinator assumes primary responsibility for monitoring client activities and insuring appropriate service delivery. This position provides collaborative services with other agencies across the county in conjunction with the Butler County Mental Health and Addiction Recovery Services Board to ensure that people who have mental health diagnosis receive the assistance they need to remain healthy and function safely in their communities.This position provides care coordination through the establishment of effective interactions with hospital staff, community members, local judges, patient/family/guardian, Butler County Children’s Services and other interdisciplinary health team members. The person in this position will have knowledge of community based resources, outpatient and inpatient resources and the treatment process for people experiencing difficulties related to their mental health.They will collaborate with other team and community providers to ensure access to needed services, with an emphasis on the transition of clients to a long-term provider within ninety (90) days or less from admission to the Connection program.
Pay:
- Starting at $18/HR, based on experience

Education: Bachelor Degree required. Preferably in human services field.
Experience: Preferably 2 years of experience working in setting serving mental health clients.
Specialized knowledge, skills, or abilities: Demonstrated knowledge about mental illness and addiction including engagement strategies, motivational strategies and knowledge and skill in the use of the Diagnostic and Statistical Manual of Mental Disorders recommended, but not required. Must have strong commitment to client rights.
