What are the responsibilities and job description for the Case Manager position at Community for New Direction?
POSITION SUMMARY:
This position will afford the opportunity to serve the adult population diagnosed with mental health and/or substance use disorders served at the outpatient treatment site. Provide direct services as needed to consumers to evaluate needs, link to resources/services, and help in identifying stability strategies. Advocates for consumers in a safe and culturally appropriate manner.
ESSENTIAL FUNCTIONS:
1. Interview and interact with clients to understand their needs and develop a client-centered treatment approach.
2. Meet the client where they are using the Stages-of-Change and Stages-of-Treatment approach.
3. Advocate for the client through assisting them in making informed decisions regarding their health, wellness and choice of treatment.
4. Have a working knowledge of the process to apply for housing, DJFS benefits, Social Security benefits as well as accessing community resources for the needs of the client.
5. Develop and maintain professional working relationships with clients while engaging, using a person-centered approach.
6. Assist clients in accessing social services and other community services.
7. Meet with clients one-on-one in the office, community or within a group setting.
8. Provides transportation services as needed.
9. Assist with applying and maintain benefits such as health insurance, food stamps, and other necessary supports.
10. Monitor and keep accurate and timely record of client treatment plan and response of client to treatment interventions.
11. Collaborate with clinicians and healthcare providers to develop and implement treatment plans.
12. Assist individuals to build personal protective support and develop healthy community support.
13. Maintain constant communication with clients to obtain feedback and ensure satisfaction with healthcare and case management services.
14. Coordinate and oversee the discharge and/or referral of clients, as necessary.
15. Prepare and present report of case management activities to supervisor and other team members.
16. Ensure compliance with fundamental case management protocols, policies, and procedures.
- Attend educational programs, conferences, and workshops to improve existing job knowledge, broaden your horizons and obtain continued education.
- Ensure that you meet weekly productivity requirements for the position.
- Performs other duties as assigned to promote the overall mission and vision of CND.
SKILLS AND ABILITIES:
Education: Associate’s degree in human services or behavior health. Bachelor’s degree preferred. Two-years’ experience in human services or behavioral science with Two years of experience in the field of mental health and/or substance use treatment.
Additional Eligibility Qualifications:
1. Good to excellent spelling, grammar, and written communication skills.
2. Must maintain CPR & First Aid Certification
3. High level of interpersonal skills to handle sensitive and confidential situations and documentation.
- Must be able to drive, have own transportation and transport consumers at times in own vehicle.
- Must attend weekly meetings
- scheduled through Microsoft TEAMS or in person
- Based on position you may have additional trainings through Relias and outside trainers.
- Must maintain professional licenses in a status. (If Applicable)
Computer Skills
Proficient in use of a personal computer and related software (e.g., Microsoft Word, Excel, PowerPoint) use of office equipment (e.g., fax, copier, printer).
Other Requirements
Must possess a valid Ohio driver's license; must pass a criminal background check and a pre-employment drug screen.
