JOB DESCRIPTION: Community Case Manager

- Position Overview (General Duties, Responsibilities and Reports) The Community Case Manager focuses on the needs of the community members. This position will be a hybrid position with time in the community and at the Community Free Clinic.
- Essential Job Functions
- Oversight of development and implementation of community-based approach to assist patients and potential patients in Washington County connect with health care services.
- Assessing clients’ physical and mental wellness, needs, preferences and abilities and using these to develop tailored care plans
- Working with clients, family and friend support networks and health care professionals to put care plans in place
- Listening to clients’ concerns and providing support or intervention as required
- Recording clients’ progress including charting referrals, home visits and other notable interactions
- Evaluating clients’ progress periodically and adjusting as needed to improve outcomes
- Following up with discharged clients to ensure they are satisfied with their services, and they are still in good physical and mental health
- Attend community events to engage potential patients and members of the community. These happen as scheduled that will include some evenings and weekend times.
- Patient Education; Provide or schedule Information relating to health issues, services in area, referral process, social determinants of health issues, etc…
- Work in Medical / Information Clinics as assigned. Assist Providers as needed with Patients and with Clinic Operation / Procedures.
- Screen Patients by verifying Patient Information and Vital Signs for Providers and Med Clinics
- Perform EKGs, Blood Pressure Checks, Draw Blood, etc. as needed
- Perform diagnostic tests as ordered; Urinalysis, Urine Pregnancy, Rapid Strep, etc.
- Obtain and Maintain CPR Certification/ First Aid Certification
· Adhere to and support the policies as mandated in the Employee Manual and other regulation requirements and the mission of the Community Free Clinic.
- Schedule Specialty Referrals and track assigned, and appointments as needed
- Aftercare follow up communication with patients or prospective patients.
- Supports the mission, philosophy, goals, and objectives of CFC.

· Maintains respect for employees and volunteers, and their individual commitment and contributions in support of CFC’s mission to provide consistent quality services.
· Performs job responsibilities with attention to safety concerns relating to staff, equipment, and facilities.
· Maintains confidentiality of patient and departmental information.
· Knowledge in the development, coordination, and expansion of patient services and programs.
· Knowledge of creating and updating spreadsheets.
· Knowledge of outside resources within the community.
· Utilizes time management within their position to assist with the needs of others: Sustained Support.
Education / Work Experience Requirements (Training Required to Perform Essential Job Functions)
· A State of MD Licensed Practical Nurse Licensure is required to perform these tasks.
· Preferred: Bilingual applicants (Spanish) or willingness to learn another language.
· This position is a hybrid position that will be based in the community and at the Community Free Clinic applicants will need to have reliable transportation, no patient transportation required.

Schedule: Hybrid: Community Events are at times scheduled evenings and weekends, so a hybrid schedule would be created to flex your work week to attend to these community opportunities.
