About the Position

As a key member of our staff, you will be responsible for delivering high-quality services that promote growth, change, and empowerment.
Key Responsibilities
- Provide individualized support and guidance to consumers, developing and implementing service plans that meet their unique needs.
- Conduct home visits, community outreach, and office meetings to ensure regular contact with consumers and stakeholders.
- Link consumers with community resources, assisting them in accessing services and supports that enhance their overall well-being.
- Maintain accurate and comprehensive records of consumer interactions, progress, and outcomes.
Requirements and Qualifications
- Bachelor's degree in a relevant field, such as social work, psychology, or sociology.
- 5 years of experience in case management, social work, or a related field.
- Act 33 & 34 clearances and FBI clearance required.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
- Strong organizational and time-management skills, with the ability to prioritize tasks and manage multiple responsibilities.
