EXECUTIVE DIRECTOR

SUMMARY

The position of the Community Health Access Project (CHAP) Executive Director is to provide leadership and overall management for the organization’s strategic direction and leading business decisions to drive growth and sustainability as a certified Pathways Community HUB (PCH). Effective service to the community is measured by strong relationships, community impact strategies management, and sound operations. The Executive Director manages relationships with stakeholders and ensures alignment of strategic and operational activity with the organization’s mission and vision, as defined by the Board of Directors.

ESSENTIAL RESPONSIBILITIES

· Maintains fidelity to the Pathways Community HUB Institute® Model (PCHI Model) and ensures that CHAP continues as a certified Pathways Community HUB.

· Responsible for grant procurement and sustainability by offering ideas on grants to the Board of Directors, and overseeing grant administration and reporting,

· Leads the overall strategic direction of the PCH that contracts with community entities for care coordination services and Medicaid Managed Care Providers and other funders to pay for care coordination services for at-risk populations ranging from pregnant women to children to adults.

· Markets the PCHI Model approach to potential agency partners within a six-county radius with the goal of developing a strong PCH Network.

· Serves as the face of the organization by being the main spokesperson and liaison with the media and is the representative of the PCH on local, state and national groups.

· Participates in external events to increase visibility credibility and develop strategic partnerships to further increase community awareness.

· Collaborates with Board members to identify, create and implement strategic plans to ensure the mission is fulfilled through programs, funding, and community outreach.

· In charge of fiscal oversight of the PCH by preparing annual organization budget in tandem with the Fiscal Officer and Finance Committee; operate within the approved budget to ensure maximum resource utilization; oversight and final approval of invoicing; and read and understand financial documents, including balance statements and profit & loss statements, as well as 990 tax returns.

· Act as an authorized signatory on all bank accounts and credit cards. Sign all Memorandums of Understanding (MOU), contract agreements and other instruments made and entered into and on behalf of the PCH.

· Responsible for assigning and overseeing coordination of supervision of all PCH personnel.

· Development and administration of policies, standards and procedures related to personnel, including staff development, information technology, budget, program performance, physical facilities, and coaches, counsels and disciplines employees per agency policy.

· Responsible for updates to PCH policies and procedures based on changes in funding, staffing, etc.

· Experienced in recruiting, hiring and supervising staff to develop a strong work culture and talented team.

· Responsible for quality improvement of external relationships and community partnerships by serving as the vendor point of contact for external systems (Care Coordination Systems), Medicaid Managed Care Plans, and other funders.

· Oversee reporting to ensure CHAP PCH data integrity and accuracy.

· Manages Information Technology risk related matters and manages IT/Vendor security audits with all Medicaid Managed Care Plans.

· The position will require travel to meetings and public presentations and may require evening and weekend work.

EDUCATION and EXPERIENCE

· A bachelor’s degree is required. A master’s degree in public health (MPH) or social work (MSSA) is preferred.

· Previous experience in a leadership role, preferably in an executive director environment.

· Demonstrated experience collaborating & managing staff, transparent and high integrity leadership.

· Strong organizational abilities including planning, delegating, and task facilitation.

· Able to be self-directed; position requires reliability, consistency, and a commitment to diversity.

· Proven success in working with a Board of Directors.

· Solid, hands-on management of a nonprofit budget, including analysis, decision-making and reporting

· Excellent writing and oral presentation skills, a persuasive and effective communicator with excellent interpersonal and multidisciplinary project skills, and proven ability to build & sustain productive relationships with diverse individuals, groups, organizations and communities.

· Knowledge of the social services community and familiarity with specific issues related to health-related social needs (HRSN).
