Community Health and Counseling Services (CHCS) is thrilled to announce the launch of our new Certified Community Behavioral Health Clinic (CCBHC) program! We will be providing comprehensive, integrated mental health and substance use treatment services at our Bangor, Dover, Lincoln, and Ellsworth offices.
What is a CCBHC? A Certified Community Behavioral Health Clinic (CCBHC) is designed to provide a comprehensive range of mental health and substance use services. Our goal is to improve access to high-quality care, enhance coordination of services, and ensure that individuals receive the support they need to lead healthier, more fulfilling lives.
Will be responsible for managing and coordinating comprehensive care for clients within the CCBHC framework. This role involves working collaboratively with interdisciplinary care teams to ensure clients receive the necessary services and support to improve their health outcomes and quality of life. The care coordinator provides outreach and advocacy services to persons entering the CCBHC. A Bachelor s degree in a human service- related discipline and a minimum of three years of experience in working with consumers in a community-based setting; three years’ experience as a case manager/coordinator is preferred, MHRT-C Certification or ability to be certified.
