What are the responsibilities and job description for the CCBHC Care Coordinator position at Community Health and Counseling Services?
Will be responsible for managing and coordinating comprehensive care for clients within the CCBHC framework. This role involves working collaboratively with interdisciplinary care teams to ensure clients receive the necessary services and support to improve their health outcomes and quality of life. The care coordinator provides outreach and advocacy services to persons entering the CCBHC. A Bachelor’s degree in a human service- related discipline and a minimum of three years of experience in working with consumers in a community-based setting; three years’ experience as a case manager/coordinator is preferred, MHRT-C Certification or ability to be certified.
