About CHDC
CHDC (Community Health and Dental Care, Inc.) is a quickly growing Community Health Center in Pennsylvania with locations in Pottstown, Barto, and Boyertown, providing care for all patients in our service area, regardless of insurance status or ability to pay.

Our History: Community Health and Dental Care was formed through the generous funding of the Pottstown Area Health and Wellness Foundation along with various local businesses. Our doors opened in October 2008 with 39 medical and dental patients seen that first week and we’ve grown to see thousands of medical and dental patients each year. As a non-profit community health center, we have been organized to become the patient’s medical home for preventive care and to promote overall well-being, thus reducing the number of emergency room visits.

About the Patient Case Manager Position
Patient Case Manager will advocate for assisting patients in need of medical, behavioral health, substance abuse or welfare related services to achieve the best possible outcomes while adhering to CHDC’s standards and policies to improve the patient’s overall quality of life. The Patient Case Manager is responsible for supporting CHDC patients and families in raising awareness and assisting in obtaining services provided by CHDC and social services in the community. Create an atmosphere that is receptive to other’s thoughts and concerns to provide compassionate care and effective communication between our patients and staff. Engage with community partners to expand case management services directly in community partners’ space.
Duties include but are not limited to:
- Educate staff on how to improve communications between CHDC, patients and social service agencies by sharing knowledge and best practice information through a well-developed skills transfer/training program for staff.
- Attend department staff meetings to achieve department goals.
- Manage the schedule to achieve appropriate coverage for assisting patients and attending events and meetings.
- Knowledge of community resources. Familiar with local social service agencies, government programs, and other resources that can benefit patients.
- Knowledge of public health programs such as Medicaid, Medicare, CHIP and Pennie.
- Be registered as a Community Partner on the statewide information system COMPASS to apply for public assistance programs for our patients and their families.
- Participate in the development of strategies for identifying, approaching, and providing education and self-management to patients through strong community collaboration.
- Address needs of the patient through direct service or referrals that include but are not limited to housing, financial, transportation, medical, behavioral health, social/recreational and spiritual concerns.
- Facilitate and effectively manage patient/provider communication as needed.
- Ensure that case management notes are clear and concise and system notes are placed into the EMR/EHR system on a consistent basis.
- Increase referrals to case management from all assigned CHDC Providers within your care team.
- Actively develop new collaborations with community partners based on CHDC’s needs assessment to increase referrals to CHDC.
- Perform other duties as assigned.
Requirements:
Qualifications, Education, and Knowledge
- Bachelor’s degree in the field of human services field preferred. Associates degree or certification in behavioral sciences a plus.
- Minimum two years’ experience in Public Health, Outreach, Social Work, or related field.
- This position requires travel to, but not limited to, all CHDC sites, including meetings, off-site retreats, conferences, workshops, and Outreach functions.
- Valid PA Driver’s License required.
- Maintenance of state minimum insurance requirements.
