Title: Licensed Clinical Social Worker
Reports To: Behavioral Health Program Director or Behavioral Health Coordinator
Department: Behavioral Health
Employment Status: Exempt

Position Summary: Provides a range of psychological assessment, diagnostic, therapeutic, and/or case management services for individuals, children, couples, groups, and/or families, as appropriate to position. Will participate in case consultation and present cases at clinical meetings.
Essential Functions & Responsibilities:
  • Advanced Clinical diagnostic assessment/documentation skills
    • Including but not limited to psychosocial assessments, progress notes, telephone consultation, brief assessments, patient education
  • Ability to assess suicide and homicide risk potential
  • Planning for improving health services by interpreting social factors pertinent to development of the program
  • The ability to work within a Multidisciplinary Team Model
  • Individual, Group and Family therapy skills
  • Strong ability to problem solve
  • Community Collaboration skills
  • Skills to assess needs and programs accordingly
  • To practice in the scope of their training, qualifications and experience
  • Adherence to their professional code of ethics
  • Strong knowledge of and connection to community resources
  • Additional duties as requested
  • School Based Health Center (If applicable)
    • An understanding of the workings of school systems
    • Prevention and intervention skills and strategies appropriate for school settings
    • The ability to independently (with support) develop a caseload while working within the school system
Additional General Requirements: Professional positive attitude, understanding of customer service principals, trustworthiness and excellent interpersonal skills.
Job Qualifications/Requirements: To perform this job successfully, an individual must be able to perform his/her duties satisfactorily. Education: LCSW position requires a Master’s degree from an accredited program in Social Work.
Experience: Must have previous experience as an LCSW in a professional clinical setting, maintaining an EHR, while providing therapeutic services. Group and family treatment preferred but not required.
Language Skills: Must speak, write and read English proficiently. Spanish preferred but not required.
Licensure/Certification Required: LCSW position requires a Master’s degree from an accredited program in Social Work, a current license from the Connecticut State Board of Registration and passed a national or state-certified licensing exam and is CPR certified.
Teleworking: This position requires teleworking if requested.

    • Compliance with current law and policy to provide a work environment free from harassment and all illegal and discriminatory behavior.
  • Cooperates and works together with all co-workers;
  • Plans and completes job duties with minimal supervisory direction, including appropriate judgment.
  • Uses tactful, appropriate communications in sensitive and emotional situations.
  • Follows up as appropriate with supervisor and co-workers regarding reported complaints, problems and concerns.
  • Promotes positive public relations with co-workers, patients, family members and guests.
Physical Effort/Environment:
Physical Demands- (More than 4 hours per day)-Requires walking, bending, sitting, standing, writing, reading, telephone use, data input into computer, and high level of audio and visual acuity. Moderate physical activity. Requires handling of average-weight objects up to 25 pounds.
Manual Dexterity Required-Ability to use a keyboard, computer, and telephone and other duties that require fine motor skills.
Virtual Environment-Proficient in utilizing virtual meeting platforms (Teams, Zoom, etc) for CHWC-related meetings, training, etc.

Understands the interests and important concerns of others. Notices and accurately interprets what others are feeling, based on their choice of words, tone of voice, expressions, and other non-verbal behavior. Understands the unspoken meaning in a situation. Says or does things to address others’ concerns.
Customer Orientation – The ability to demonstrate concern for satisfying one’s external and/or internal customers. Quickly and effectively solves customer problems. Talks to customers to find out what they want and how satisfied they are with what they are getting. Lets customers know he/she is willing to work with them to meet their needs. Helpful, positive manner with peers, subordinates and customers.
