Summary:
Social Work services at Middlesex Hospital encompass a wide scope of assistance for patients and families. The primary focus is to work directly with the patients and families to assess where they are in their process of understanding the diagnosis/prognosis and coping. We promote communication between the patient, family and health care providers. A comprehensive psychosocial assessment is completed by social work to identify needs and goals. Social work then works collaboratively with the patients and families toward achievement. Services provided by social work clinicians include but are not limited to;
Responsibilities:
- Supportive counseling, crisis intervention, teaching regarding disease progression, advanced directives, and end of life planning
- Knowledge and referrals to community resources, discharge planning
- Access/education regarding entitlements and assistance with completion of entitlements
- Financial stressors and insurance coverage
- Facilitate family meetings
- Active members on an interdisciplinary team
Qualifications:
- Hold a master’s degree from a social work program accredited by the Council on Social work Education
- State of Connecticut License in Clinical Social Work preferred
- Good analytical, organizational and interpersonal skills
- Must be able to communicate effectively orally and in writing.
- Must be able to work collaboratively with other members of the health care team
- Computer Literacy
